Bad breath: real causes and solutions

Bad breath (halitosis) affects 25% of adults. The 7 most frequent causes are: bacteria on the tongue, gingivitis, active cavities, dry mouth, infection in a previous root canal, digestive issues and diet. The good news: 85% resolves with professional hygiene + tongue scraper + hydration. The other 15% requires specific dental treatment — a check-up identifies the exact cause.

August 6 is National Fresh Breath Day in the United States, a date that uses summer to bring visibility to something many people suffer in silence: halitosis. It's a delicate topic, loaded with shame, but rarely addressed with clinical seriousness. Here we explain the real causes and what truly works.

The 7 most frequent causes

1. Bacteria on the tongue

Cause #1 of bad breath. The posterior dorsum of the tongue accumulates anaerobic bacteria that produce volatile sulfur compounds (VSCs). Solution: daily tongue scraper + tongue brushing.

2. Periodontal disease

Gingivitis and periodontitis harbor bacteria in periodontal pockets that produce constant odor. Solution: professional periodontal treatment + rigorous hygiene.

3. Cavities and defective restorations

Cavities and leaking fillings harbor bacteria and fermented residue. Solution: restore cavities, replace compromised fillings.

4. Endodontic infections

A tooth with pulp necrosis or an abscess produces a characteristic foul odor. Sometimes without pain but with persistent halitosis. Solution: root canal or retreatment.

5. Dry mouth (xerostomia)

Without enough saliva, bacteria proliferate. Causes: medications, diabetes, stress, mouth breathing. Solution: hydration, saliva substitutes, treat the underlying cause.

6. Systemic problems

Uncontrolled diabetes (sweet ketone breath), kidney disease (ammonia breath), gastroesophageal reflux (acidic breath), chronic sinusitis. Solution: medical care for the underlying condition.

7. Diet and habits

Prolonged fasting, ketogenic diets, smoking, alcohol, garlic/onion. Solution: hydration, moderation and habit management.

What does NOT work

What DOES work, in order of priority

  1. Correct brushing twice a day with proper technique.
  2. Daily interproximal cleaning (floss or interdental brushes).
  3. Tongue scraper as part of the nighttime brushing.
  4. Abundant hydration throughout the day.
  5. Professional cleaning every 6 months.
  6. Treatment of pending cavities, periodontitis or root canals.
  7. Medical evaluation if everything dental is healthy and it persists.

How do you know if you really have bad breath?

Self-assessment is hard because the sense of smell adapts to your own odor. Useful methods: ask someone you trust, lick the back of your wrist and smell after 10 seconds, or scrape the back of your tongue with a spoon and smell the scraping.

Bad breath has a cause, and the cause has a solution

Chronic halitosis isn't "something you live with." In 90% of cases the cause is correctable. At Clínica Endodontics in San Miguel, we identify whether the cause is dental and which one specifically. Don't be embarrassed to ask — the first step of treatment is talking about it.
